What this trial studies
This clinical trial evaluates the SCThrive intervention, a behavioral self-management program designed for adolescents and young adults with sickle cell disease (SCD). Participants aged 13-21 will engage in an 8-week virtual group intervention supported by a mobile app, focusing on enhancing patient activation and self-management behaviors. The study will compare outcomes between those using SCThrive and those receiving standard care, assessing improvements in daily functioning and emergency room visits. Additionally, the research will explore the impact of social determinants of health on treatment response.

Inclusion Criteria: * Patient of a participating SCD Clinic * Confirmed diagnosis of SCD * 13-21 years of age Exclusion Criteria: * Another chronic disease (which would complicate measurement of patient activation) * Non-English-speaking * Cognitive or psychiatric disorder that the physician or study therapists believe would impair study participation.
