What this trial studies
This trial evaluates the effectiveness of an electronic health record best practice advisory system designed to alert clinicians about guideline-recommended heart failure medications that are not currently prescribed to their patients. It compares this intervention to usual care practices in outpatient clinics and hospitals within the Inova Health System. The goal is to enhance medication prescription for patients with heart failure and reduced ejection fraction, thereby improving patient outcomes.
Who should consider this trial
Good fit: Ideal candidates are adults over 18 years old with heart failure and a left ventricular ejection fraction of 40% or less who are seen by a cardiologist at Inova.
Not a fit: Patients receiving hospice care, those who have opted out of EHR-based research, or those already on maximally titrated doses of heart failure medications may not benefit from this study.

Inclusion Criteria: * Age\>18 Years * Seen by a Cardiologist in an Inova clinic or Inova hospital setting * Left Ventricular Ejection Fraction ≤40% Exclusion Criteria: * Opted out of EHR-based research * Hospice care * Already receiving maximally titrated doses of each of the four classes of evidence- based HFrEF medical therapy * Prior heart transplant
