What this trial studies

This pilot feasibility study aims to evaluate a clinical decision support tool designed for pharmacists to enhance adherence to guideline-directed medical therapy (GDMT) for patients with heart failure with reduced ejection fraction (HFrEF). The study utilizes the RE-AIM evaluation framework to assess the tool's reach, effectiveness, adoption, implementation, and maintenance. By addressing gaps in care quality, the study seeks to improve patient outcomes through better medication management and coordination in pharmacist-led clinics. The focus is on optimizing treatment for patients who have already undergone intensive disease management programs.

Who should consider this trial

Good fit: Ideal candidates for this study are pharmacists working at Northwestern Medicine who are involved in the Medication Adjusted to Target (MAT) Clinic.

Not a fit: Patients who are not pharmacists or those not participating in the MAT Clinic will not benefit from this study.

Why it matters

Potential benefit: If successful, this approach could significantly improve medication adherence and health outcomes for patients with heart failure.

How similar studies have performed: Previous studies have shown that pharmacist-led clinics can effectively improve adherence to guideline-directed medical therapy, indicating potential success for this approach.
