This test will see if hypopressive breathing exercises or transversus abdominis-focused core training better reduces urinary leakage and improves pelvic floor function and quality of life in postpartum women.

What this trial studies
Postpartum women with stress urinary incontinence are assigned to either a hypopressive breathing/exercise program or a transversus abdominis-focused core training program delivered over four weeks. Urinary incontinence severity, pelvic floor function, and quality of life are measured at baseline and after 4 weeks using the ICIQ-UI, King's Health Questionnaire (KHQ), and IIQ-7. Data will be analyzed using SPSS version 25 to compare changes between the two intervention groups. The study excludes participants with neurological impairments, connective tissue disorders, recent urinary or vaginal infection, detrusor instability, prior SUI surgery, or pelvic organ prolapse.
Who should consider this trial
Good fit: Multiparous women who had an uncomplicated normal vaginal delivery, have stress urinary incontinence confirmed by a positive pad test, and no medical or gynecological risk factors are ideal candidates.
Not a fit: Women with neurological impairment or connective tissue disorders, active urinary or vaginal infection in the past three months, known detrusor instability, prior surgery for SUI, or pelvic organ prolapse are unlikely to benefit from these interventions.
Why it matters
Potential benefit: If successful, one of the exercise programs could reduce postpartum urinary leakage and improve pelvic floor function and everyday quality of life.
How similar studies have performed: Exercises targeting the transversus abdominis and pelvic floor have shown benefit in reducing postpartum urinary incontinence in prior studies, while hypopressive training is less well established and has produced mixed results.

Inclusion Criteria: * Multiparous women with urinary incontinence * Who had undergone normal vaginal delivery * Free from any medical and gynecological risk factors and /or conditions * Stress urinary incontinence test is positive: Pad test Exclusion Criteria: * Had neurological impairments or connective tissue disorders * Active urinary or vaginal infection in the past 3 months * Had known detrusor instability * Had undergone previous surgery to alleviate SUI * Had pelvic organ prolapse
