This project will try a host-led harm reduction approach to reduce substance-related, sexual, mental health, and legal harms among men who have sex with men who engage in chemsex in Bangkok.

What this trial studies

HAMMOCK is a cross-sectional mixed-methods pre-implementation study using the EPIS framework to explore chemsex practices among MSM hosts and guests in Bangkok. The study combines self-administered questionnaires and in-depth interviews with hosts, guests, and stakeholders including community organizations, clinic staff, and police to map harms, contexts, and needs. Investigators will co-develop and refine a host-led harm reduction model and identify barriers and facilitators to implementation. Results will inform preparation for pilot implementation of tailored harm reduction strategies in private chemsex settings.

Who should consider this trial

Good fit: Ideal participants are Thai-speaking men who have sex with men aged 18 or older who have hosted or attended at least two chemsex events in the past six months, or community organization, hospital, or police staff with at least three months' relevant experience.

Not a fit: People who are not MSM, under 18, unable to consent or communicate in Thai, not involved in chemsex, located outside Bangkok, or currently needing urgent medical or addiction treatment are unlikely to benefit from this pre-implementation work.

Why it matters

Potential benefit: If successful, the model could offer practical, host-delivered strategies that lower overdose risk, improve sexual and mental health outcomes, and reduce socio-legal harms for MSM involved in chemsex.

How similar studies have performed: Community-led harm reduction programs have shown benefits in related substance-use and sexual-health contexts, but a host-led model specifically targeting chemsex is largely novel and has limited prior testing.

Inclusion Criteria:

* MSM aged ≥18 years
* Able to communicate in Thai
* Specific by group:

  * Chemsex hosts: hosted ≥2 times in past 6 months
  * Chemsex guests: attended ≥2 times in past 6 months
  * CSO / hospital / police staff: working ≥3 months in related roles
* Provide informed consent

Exclusion Criteria:

• Unable to provide informed consent (e.g., under substance influence at consent)
