What this trial studies

This project aims to improve access to cardiac rehabilitation (CR) for individuals living with heart failure (HF) and their caregivers by implementing a home-based program called DK: REACH-HF. The program is adapted from a successful UK initiative that has shown improvements in quality of life and cost-effectiveness. By addressing barriers associated with traditional center-based CR, such as travel and group exercise discomfort, this study seeks to enhance participation rates among HF patients in Denmark. The intervention will involve self-care support tailored to the needs of patients and caregivers.

Who should consider this trial

Good fit: Ideal candidates for this study are adults over 18 years old with a confirmed diagnosis of heart failure with reduced ejection fraction who are stable and suitable for cardiac rehabilitation.

Not a fit: Patients who have participated in cardiac rehabilitation within the past 12 months or those unable to understand the study information will not benefit from this study.

Why it matters

Potential benefit: If successful, this program could significantly improve the quality of life for heart failure patients and reduce hospital readmissions.

How similar studies have performed: Previous studies using home-based cardiac rehabilitation programs have shown success in improving patient outcomes, indicating a promising approach.
