What this trial studies
This project aims to improve access to cardiac rehabilitation (CR) for individuals living with heart failure (HF) and their caregivers by implementing a home-based program called DK: REACH-HF. The program is adapted from a successful UK initiative that has shown improvements in quality of life and cost-effectiveness. By addressing barriers associated with traditional center-based CR, such as travel and group exercise discomfort, this study seeks to enhance participation rates among HF patients in Denmark. The intervention will involve self-care support tailored to the needs of patients and caregivers.
Who should consider this trial
Good fit: Ideal candidates for this study are adults over 18 years old with a confirmed diagnosis of heart failure with reduced ejection fraction who are stable and suitable for cardiac rehabilitation.
Not a fit: Patients who have participated in cardiac rehabilitation within the past 12 months or those unable to understand the study information will not benefit from this study.

Eligibility criteria

Inclusion Criteria: * Aged \> 18 years * Confirmed diagnosis of Heart Failure (HF) with reduced Ejection Fraction (HFrEF) (Ejection Fraction (EF) \<45%) * No worsening of HF symptoms within past 2 weeks resulting in hospitalization (to ensure stable patients) * Are deemed suitable for participating in Cardiac Rehabilitation (CR) by the outpatient HF clinic Exclusion Criteria: * Have undertaken CR within the past 12 months * Are unwilling/unable to travel to a rehabilitation center in the municipality or at the hospital * Are unable to understand the study information and/or corporate on giving informed consent
