What this trial studies
This pilot study evaluates the effectiveness of the Self-Empowerment Care Model in enhancing independence and quality of life among frail older adults in Hong Kong. The intervention, based on Nordic re-enablement principles, focuses on hydration, exercise, diet, and defecation care over a 12-week period. It employs a quasi-experimental design with 60 participants, comparing outcomes between an intervention group and a control group receiving standard care. Additionally, the study assesses the impact on caregivers' burden and job satisfaction.
Who should consider this trial

Not a fit: Patients with severe cognitive impairment, severe mental illness, or terminal illness may not benefit from this study.

Inclusion Criteria: * Age 65 years or older * Functional decline * Potential to benefit from reablement. Exclusion Criteria: * Severe cognitive impairment, those with mild impairment will be considered. * Severe mental illness * Terminal illness
