This study will test whether a daily herbal supplement containing Boswellia serrata, turmeric (curcumin), and grape extract (Vitis vinifera) reduces pain and improves function in adults with hip or knee osteoarthritis.

What this trial studies
This is a randomized, placebo-controlled crossover trial in which adults with clinically diagnosed hip or knee osteoarthritis will take the active herbal supplement for four weeks and a matching placebo for four weeks, one after the other. Participants will complete validated symptom questionnaires and three performance-based physical tests six times in total and will provide a urine sample for safety or compliance checks. Eligible participants must have moderate pain (NRS ≥4) and functional impairment (Lequesne Index ≥7); people with autoimmune disease, recent steroid injections, severe joint damage, pregnancy, or abnormal liver/kidney function are excluded. The primary comparison is symptom change between the supplement and placebo periods.
Who should consider this trial
Good fit: Adults aged 18 and over with clinically diagnosed knee or hip osteoarthritis, moderate pain (NRS ≥4) and a Lequesne Functional Index ≥7 who can attend the London site and complete online assessments.
Not a fit: People who are pregnant or breastfeeding, have autoimmune joint disease, recent intra-articular or systemic corticosteroid use, severe joint injury or complete cartilage loss, significant liver or kidney dysfunction, or known allergy to the supplement ingredients are unlikely to benefit or be eligible.
Why it matters
Potential benefit: If effective, the herbal combination could reduce pain and improve mobility for people with knee or hip osteoarthritis, offering a non-prescription alternative to standard pain medications.
How similar studies have performed: Multiple small randomized trials and meta-analyses have reported modest symptom improvements with curcumin and Boswellia formulations in osteoarthritis, but larger, rigorously controlled studies are still limited.

Inclusion Criteria: * Aged ≥ 18 * Diagnosis of osteoarthritis in knee or hip * Numerical Rating Scale (NRS) ≥ 4 during the most painful movement in the last 24 hours * Lequesne's Functional Index (LFI) score ≥ 7 * Ambulant patient Exclusion Criteria: * Pregnant and breastfeeding * Autoimmune disease such as rheumatoid arthritis, gout, lupus * Joint trauma, joint injury, joint infection, meniscus tear, complete loss of articular cartilage * Expectation of surgery * History of the viscous or corticosteroids injections into affected joints or oral corticosteroids within last 12 months * Allergy to one of the intervention's ingredients or NSAIDs * Peptic ulceration and upper gastrointestinal haemorrhage * High alcohol intake, inability to abstain from alcohol, substance abuse, history of addiction * Tumor, cancer * Abnormal renal or/and hepatic functions or altered blood chemistry * Use of concomitant medication able to interfere with the interventions
