What this trial studies
This study aims to evaluate the effectiveness of a patient navigator in improving pain management and daily functioning for individuals with chronic pain. Participants will engage with a web-based cognitive behavioral therapy program called PainTrainer, which consists of eight modules designed to teach coping strategies for pain management. Additionally, some participants will receive personalized support through phone calls with a patient navigator to help coordinate their care and address social determinants affecting their health. The study focuses on patients who experience chronic musculoskeletal pain and face social challenges that may impact their health outcomes.
Who should consider this trial
Good fit: Ideal candidates are individuals experiencing daily chronic musculoskeletal pain for at least three months and facing social challenges such as financial insecurity or lack of social support.
Not a fit: Patients with active cancer, severe cognitive impairments, or those already receiving similar support from a navigator or social worker may not benefit from this study.

Eligibility criteria

Inclusion Criteria: * Patients with daily pain for 3 months or longer * Chronic daily musculoskeletal pain with Pain, Enjoyment, General Activity (PEG) Survey score \>=4 * Primary care provider must be an Atrium provider * Has at least one social driver of health: Financial insecurity, work and income, housing, food insecurity, safety, health literacy and at least one of the above, lack of social support and at least one of the above Exclusion Criteria: * Patients who have active cancer * Cognitive impairment severe enough to preclude participation in a behavioral/lifestyle change program * Live in a nursing home or inpatient treatment facility * Unable to read and converse in English * Those who are already connected to a patient navigator or nurse coordinator or social worker * Planned elective surgery in the next 6 months
