This study will test whether adding a structured exercise program to the Healthworks! weight-loss program helps lower blood fat levels and improve blood-vessel function in 10–20-year-olds with obesity.

What this trial studies
Participants who are starting the Healthworks! structured weight-loss program will follow an exercise intervention and wear a Garmin Vivosmart tracker for one week at a time on three occasions during the six-month study. The team will collect device activity data, perform laser Doppler blood-flow testing, and use a Veggie Meter to estimate vegetable intake at baseline and at six months, alongside routine blood draws and clinical assessments from the Healthworks! visits. Text-message reminders will be sent for visit scheduling and daily at-home procedure prompts, and study visits are expected to last about an hour and may be scheduled separately from clinical visits. The protocol focuses on changes in blood lipid levels and endothelial (blood-vessel) function over the study period.
Who should consider this trial
Good fit: Ideal candidates are 10–20 years old with obesity (BMI ≥ 30 kg/m2 or BMI ≥ 95th percentile for age and sex) who plan to start the Healthworks! structured weight-loss program and speak English or Spanish.
Not a fit: Patients who do not meet the BMI or age criteria, are not joining the Healthworks! program, or who cannot complete the exercise program or study visits are unlikely to benefit from this protocol.
Why it matters
Potential benefit: If successful, the program could reduce blood lipid levels and improve blood-vessel function, which may lower long-term cardiovascular risk in youth with obesity.
How similar studies have performed: Prior exercise programs in adolescents have produced modest improvements in blood lipids and endothelial function, though results across studies have been mixed.
