What this trial studies
This clinical trial evaluates a pharmacist-led Medication Management Service (MMS) aimed at optimizing medication use and improving health outcomes for patients with Type 2 diabetes mellitus and hypertension in community pharmacies across Hong Kong. The study involves a randomized controlled design where participants will receive MMS after a control period without the service. The trial will assess patient satisfaction, medication adherence, health-related quality of life, and overall health outcomes. A total of 640 participants will be recruited, and their clinical data will be analyzed to determine the effectiveness and cost-effectiveness of the MMS.
Who should consider this trial
Good fit: Ideal candidates are adults aged 18 and above diagnosed with Type 2 diabetes mellitus and/or hypertension who are taking multiple chronic medications.
Not a fit: Patients who are not fluent in Cantonese or English, or those unable to provide informed consent, may not benefit from this study.

Eligibility criteria

Inclusion Criteria: * aged 18 years or above * diagnosed with Type 2 diabetes mellitus and/or hypertension * having regular follow-up on Type 2 diabetes mellitus and/or hypertension at Hospital Authority * no A\&E admission/hospitalization in the past 3 months * no recent changes in medication regimen in the past 3 months * polypharmacy (taking 5 or more chronic medications) * using at least 1 high-risk medication (Anticoagulants, Oral hypoglycaemics, Insulins, psychotropic medications or immunosuppressants) * able to communicate in Cantonese and/or English Exclusion Criteria: * aged below 18 * unable to give informed consent * unable to communicate in Cantonese and/or English
