What this trial studies

This cross-sectional observational project uses an integrated Health Ecology and COM-B (Capability-Opportunity-Motivation-Behavior) framework to examine multilevel determinants of medication adherence in adults with diabetes. Participants who are prescribed long-term medicines complete structured questionnaires about cognitive factors, treatment beliefs, family and social support, patient–provider communication, and practical access issues such as cost and distance to care. The study analyzes how intrapersonal, interpersonal, healthcare interaction, and structural factors interact to shape medication-taking behavior. Findings will be used to identify the most important barriers and supports to inform patient-centered strategies for improving adherence.

Who should consider this trial

Good fit: Adults aged 18 or older with diabetes who have been on long-term medication, can understand and complete questionnaires, and can give informed consent are ideal candidates.

Not a fit: People with acute medical emergencies, severe cognitive or psychiatric impairment, terminal illness, or those unable/unwilling to provide consent or complete questionnaires are unlikely to benefit from participation.

Why it matters

Potential benefit: If successful, the findings could guide more targeted, patient-centered approaches that improve medication-taking and diabetes control.

How similar studies have performed: Prior research has repeatedly found multilevel barriers to adherence and COM-B has been used successfully in related adherence work, though fully integrated ecological-COM-B approaches remain relatively novel.

Eligibility criteria

Inclusion Criteria:

* Adults aged 18 years or older
* Diagnosed with at least one chronic disease (e.g., diabetes mellitus, hypertension, cardiovascular disease, chronic respiratory disease, chronic kidney disease) for a minimum duration of six months
* Prescribed long-term medication therapy
* Able to understand and respond to the study questionnaire
* Willing to provide written informed consent

Exclusion Criteria:

* Patients with acute medical conditions requiring immediate care
* Individuals with severe cognitive impairment or psychiatric illness that would preclude reliable participation
* Patients who are terminally ill
* Patients unwilling to participate or provide consent
