What this trial studies

This observational study follows adults with advanced chronic kidney disease who are transitioning to dialysis, either at home or in a hospital setting. It aims to assess changes in patients' quality of life and frailty status before and up to 12 months after starting dialysis. By utilizing a mixed-methods approach, the study will gather both quantitative and qualitative data to understand the experiences of patients and caregivers during this transition. The research will take place across 12 centers in Canada, engaging patients and caregivers actively throughout the process.

Who should consider this trial

Good fit: Ideal candidates are adults with advanced chronic kidney disease who have chosen their dialysis modality and have an estimated glomerular filtration rate of less than 12 mL/min/1.73m2.

Not a fit: Patients who are oriented toward conservative treatment or those with a planned kidney transplantation within 6 months may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

1. Adults followed in advanced CKD clinic,
2. estimated glomerular filtration rate (eGFR) \<12 mL/min/1.73m2 103,
3. Have chosen their dialysis modality,
4. Understand English or French.

Exclusion Criteria:

1. Orientation toward conservative treatment,
2. Planned kidney transplantation \< 6 months,
3. Unable to provide consent due to severe cognitive or psychiatric disease,
4. Previous treatment with dialysis \> 3 months,
5. Life expectancy \< 6 months
