What this trial studies

This randomized clinical trial compares five non-surgical periodontal protocols—including scaling and root planing, air polishing, adjunctive 810 nm diode laser, and antibiotic prophylaxis—to measure their effects on systemic inflammatory burden. Participants with advanced periodontitis will be randomly assigned to one of the treatment arms and undergo standardized mechanical debridement with or without the specified adjuncts. Blood samples will be collected at baseline and at follow-up visits to measure inflammatory biomarkers such as CRP and IL-6, and clinical periodontal outcomes and bacteremia will be monitored. The trial excludes heavy smokers, recent antibiotic or periodontal treatment recipients, pregnant or lactating people, and those on medications that alter inflammatory markers.

Who should consider this trial

Good fit: Adults with stage III or IV periodontitis who are non-smokers or light smokers (<10 cigarettes/day), free of systemic diseases or medications that affect inflammatory markers, not pregnant or breastfeeding, and without recent antibiotics or periodontal treatment are ideal candidates.

Not a fit: People with mild periodontitis, heavy smokers, those taking medications or with systemic conditions that alter inflammatory markers, recent periodontal treatment, or pregnancy/lactation are unlikely to qualify or benefit from the specific outcomes measured.

Why it matters

Potential benefit: If successful, the findings could identify treatment approaches that reduce systemic inflammation linked to periodontitis and help guide safer, more effective non-surgical care.

Inclusion Criteria:

* Periodontitis stage III or IV
* Non-smokers or light smokers (\<10 cigarettes/day)
* No NSAIDs in regular basis or antibiotics 3 months before
* No previous periodontal treatment 12 months before
* No presence of other acute or chronic infections
* No systemic disease or medication known to affect the serum level of inflammatory markers (cyclooxygenase inhibitors, platelet aggregation inhibitors, lipid lowering agents, â-adrenoreceptor antagonists, angiotensin converting enzyme inhibitors, antidiabetic agents, estrogen-based medications, medication for autoimmune disease, magnesium or vitamin E supplements)
* No pregnancy or lactation
* Written informed consent.
