CHEST-CA: Study of Chest Pain and Hidden Cardiac Amyloidosis

We will test whether men over 65 who have chest pain but no coronary artery disease might have hidden cardiac amyloidosis using blood and urine tests, ECG and echocardiography, bone scintigraphy, and sometimes a heart biopsy.

What this trial studies

This is a prospective observational study enrolling men older than 65 who present with chest pain but have no evidence of coronary artery disease on CT coronary angiography or Rb-PET. Participants will undergo blood and urine sampling, ECG, transthoracic echocardiography, and bone scintigraphy to look for signs of cardiac amyloid, with endomyocardial biopsy reserved for inconclusive cases. The primary aim is to determine the prevalence of cardiac amyloidosis in this defined chest-pain population. Data collected at Rigshospitalet will inform whether routine screening in similar patients is warranted.

Who should consider this trial

Good fit: Men over 65 with chest pain who have had a CT coronary angiogram or Rb-PET showing no coronary artery disease and who do not have a prior history of amyloidosis.

Not a fit: Patients who are female, younger than 65, have confirmed coronary artery disease on imaging, or already have a diagnosis of amyloidosis are unlikely to benefit from this screening-focused study.

Why it matters

Potential benefit: If successful, the study could identify men with previously unrecognized cardiac amyloidosis earlier, enabling earlier management and appropriate follow-up.

How similar studies have performed: Noninvasive bone scintigraphy for transthyretin cardiac amyloidosis has been validated and prevalence studies in older adults with heart failure or aortic stenosis have shown notable rates, but applying this approach specifically to chest-pain patients without CAD is relatively novel.

No signs of CAD through Rb-PET and/or CT coronary arteriogram

Exclusion Criteria:

Previous history of amyloidosis
