This registry will collect information from adults with type 2 diabetes to see how their heart risk and medicines are managed in real-world Italian outpatient clinics.

What this trial studies
CardioMET is an observational, real-world registry enrolling adults with type 2 diabetes from internal medicine and cardiology outpatient clinics in Italy. It will collect clinical and pharmacological data to characterize patients' cardiovascular risk profiles and document management according to current ESC and AHA prevention guidelines. Eligible participants are adults with stable T2DM who can provide written informed consent, while type 1 diabetes, active participation in interventional trials, or conditions that increase participation risk are excluded. The collected data will be analyzed to identify patterns of care and gaps in guideline implementation to inform future quality-improvement efforts.
Who should consider this trial
Good fit: Adults (≥18) with stable type 2 diabetes seen in internal medicine or cardiology outpatient clinics in Italy who can give written informed consent are ideal candidates.
Not a fit: People with type 1 diabetes, those who are hospitalized or medically unstable, patients already enrolled in interventional studies, or those unable to consent are unlikely to benefit from participating.

Inclusion Criteria: * Age ≥ 18 years; * Diagnosis of type 2 diabetes mellitus (T2DM); * Stable clinical conditions * Outpatient settings * Ability to understand the study procedures and provide written informed consent. Exclusion Criteria: * Patients with type 1 diabetes mellitus; * Active participation in interventional studies; * Any condition that may put the patient at risk by participating in the study.
