What this trial studies
The PREVENT program aims to reduce the incidence of hip fractures among older adults in long-term care facilities by utilizing a fracture risk calculator based on residents' electronic health records. This multifaceted approach involves training healthcare teams, including doctors, pharmacists, and nurses, on the latest recommendations for fracture prevention and treatment decision-making. The program also provides tools and templates to help healthcare teams implement strategies to reduce falls and fractures effectively. By focusing on person-centered care, the initiative seeks to improve the quality of life for residents at risk of fractures due to osteoporosis and falls.
Who should consider this trial
Good fit: Ideal candidates for this program are older adults residing in long-term care facilities who are at risk of fractures due to osteoporosis.
Not a fit: Patients with end-stage diseases, those who are comatose, or receiving hospice care may not benefit from this program.
Why it matters
Potential benefit: If successful, this program could significantly reduce the occurrence of hip fractures and improve the overall health and independence of older adults in long-term care.
How similar studies have performed: Other studies have shown success with similar multifaceted approaches to fracture prevention in older adults, indicating a promising avenue for reducing fracture rates.
