The COPD Patient-Powered Research Network (COPD PPRN) aims to enroll over 75,000 patients with Chronic Obstructive Pulmonary Disease (COPD) and those at risk, allowing them to share their health information over several years. This observational study utilizes an online platform for volunteers to enroll, complete surveys, and be contacted about qualifying research studies. Inclusion Criteria:

* 18 years or older that says yes to one of the following:
* COPD diagnosis
* Current or former smoker
* Family history of respiratory disease
* Symptomatic of respiratory disease

Exclusion Criteria:

* Non-English speaking
