This trial will test whether regular video calls after hospital discharge help people with HFrEF manage their condition and self-care better than audio-only calls.

What this trial studies

This is a randomized trial comparing structured video monitoring versus audio-only monitoring after hospital discharge for patients with heart failure with reduced ejection fraction. Participants randomized to the video arm receive nurse-led video calls guided by cardiologists at 7, 30, 60, 180, and 365 days after discharge, while the control arm receives audio calls on the same schedule. Outcomes include measures of self-care and other clinical outcomes such as readmissions and adverse events. Eligible participants are adults hospitalized for decompensated HFrEF with ejection fraction <40% who have access to an internet-capable mobile device.

Who should consider this trial

Good fit: Adults (≥18) recently hospitalized for decompensated HFrEF with an ejection fraction under 40% and access to a mobile device with internet are ideal candidates.

Not a fit: Patients on a heart transplant waiting list, those who had coronary artery bypass in the past 3 months, those in palliative care, those with life expectancy under one year, or those without an internet-capable device are unlikely to benefit from this intervention.

Potential benefit: If successful, this approach could improve patient self-care, reduce readmissions, and enhance post-discharge outcomes for people with HFrEF.

Eligibility criteria

Inclusion Criteria:

* Patients aged 18 years or older;
* Admitted for decompensated HFrEF
* Ejection fraction less than 40% confirmed by echocardiogram performed in the last 3 months
* Who access to a mobile device with internet access

Exclusion Criteria:

* heart transplant waiting list;
* who had undergone coronary artery bypass in the last 3 months ;
* who were in palliative care;
* who had a life expectancy of less than 1 year
