Caring for Frail Patients Through Vaccination
This project uses hospital records to find frail adults who may be missing recommended vaccines and contacts them to offer pneumococcal, zoster, influenza, hepatitis B, and COVID-19 vaccination.

What this trial studies
The project applies an automated algorithm to electronic health records and the regional vaccination registry to identify adults seen in selected departments who are missing recommended vaccines. Consenting eligible patients will be contacted in hospital, by message, or by email and offered enrollment in a clinical pathway to increase vaccination coverage. The pathway targets pneumococcal, zoster, influenza, hepatitis B, and SARS-CoV-2 vaccines and follows the national vaccination plan and recent Ministry of Health guidance. Expected outcomes include higher vaccine uptake among frail patients and fewer vaccine-preventable complications requiring hospital care.
Who should consider this trial
Good fit: Adults (18+) who have attended Dermatology, Oncological Gynecology, Geriatrics, Nephrology, or Gastroenterology at Fondazione Policlinico Universitario A. Gemelli IRCCS and have medical residence in the Lazio region are the intended candidates.
Not a fit: Patients already up to date with the recommended vaccines, those living outside the Lazio region, those who do not attend the listed departments, or those who decline contact/consent are unlikely to benefit.
Why it matters
Potential benefit: If successful, this approach could raise vaccine coverage in frail patients and reduce hospital stays, long-term complications, and deaths from these infections.
How similar studies have performed: Electronic health record–based identification and outreach programs have previously improved adult vaccine uptake in other settings, although targeting frail inpatient populations is less extensively studied.

Inclusion Criteria: * 18 years old or more, * Patients who have accessed one of the enrolled departments (Dermatology, Oncological gynaecology, Geriatry, Nephrology, Gastroenterology), * Medical residence in Lazio Region Exclusion Criteria: \- Patients who do not fulfill 1 or more inclusion criteria
