This project will test whether automated voice-message reminders that talk about vision-loss risk, combined with free reading glasses, help people with diabetes in Dinajpur, Bangladesh keep their diabetes and eye appointments and get better patient-centered care.

What this trial studies

Participants receiving care at Dinajpur diabetes services will be sent automated voice messages and offered free reading glasses as part of an intervention to encourage diabetes and diabetic retinopathy (DR) service uptake. The primary comparison is between messages that include information about vision-loss risk plus appointment reminders and messages that only give appointment reminders. The main outcome is the proportion of patients completing scheduled DM and DR appointments within three weeks of the scheduled date, with a cost-effectiveness analysis of intervention cost per additional compliant patient. Enrollment is limited to adults over 30 recently diagnosed with diabetes who are treated at Dinajpur Diabetes Hospital and can receive voice calls.

Who should consider this trial

Good fit: Adults over 30 diagnosed with diabetes within the past six months, currently treated at Dinajpur Diabetes Hospital, who have a mobile phone and can give informed consent are ideal candidates.

Not a fit: People whose vision loss is due to other major eye diseases or who have severe medical conditions that prevent follow-up (for example stroke or advanced dementia) are unlikely to benefit from this intervention.

Inclusion Criteria:

* Diagnosed with DM within the last 6 months
* Over 30 years of age
* Currently treated at the Dinajpur Diabetes Hospital
* Possess a mobile phone to receive voice messages
* Willingness to participate in the study, and ability to give informed consent

Exclusion Criteria:

* Other significant causes of vision loss
* Medical conditions that could impair compliance with follow-up (such as stroke, dementia, etc.)
