What this trial studies
This clinical trial evaluates the effectiveness of short-form health education videos, designed in a social media style, in enhancing health care transition readiness, self-efficacy, emotional well-being, health literacy, and appointment attendance among adolescents with chronic illnesses. Participants aged 12 to 17 will be randomly assigned to either the video intervention or a standard health education website and will engage with the materials for 20 minutes, followed by completing surveys that take 30-60 minutes. The study aims to compare the outcomes immediately after the intervention and at a six-month follow-up to determine the impact of the video intervention.

Inclusion Criteria: * Patients in subspecialty clinics * Between 12 and 17 years old * With access to an internet-connected device * Speaking English or Spanish * With sufficient cognitive capacity to understand the study procedures, as assessed via a teach back method. Exclusion Criteria: * none
