What this trial studies

Participants who are 1–5 years post-hematopoietic cell transplant are randomized to one of four arms: PACT website plus a 45–60 minute navigation session, navigation only, PACT only, or usual care. The PACT intervention is a four-week, web-based toolkit for advance care planning, and the navigation intervention is a single session with a trained health coach to review goals-of-care and advance directive materials. Study staff collect outcomes related to engagement in advance care planning and occurrence of goals-of-care conversations, with follow-up at 4 and 12 weeks after the intervention period. The trial requires English fluency and internet access and excludes those unable to consent or with uncontrolled psychiatric illness.

Who should consider this trial

Good fit: Adults (age 18+) who had a hematopoietic cell transplant for a blood cancer 1–5 years ago, who can read and speak English, have internet access, and have not completed advance directives or had a goals-of-care conversation are ideal candidates.

Not a fit: People without internet access, non-English speakers, those who already completed advance directives or had goals-of-care conversations, those unable to consent, or those with uncontrolled psychiatric illness are unlikely to benefit from this intervention.

Why it matters

Potential benefit: If successful, the interventions could increase completion of advance directives and improve end-of-life care that matches patient preferences for HCT survivors.

How similar studies have performed: Prior work with navigator-led ACP programs and web-based advance care planning tools has shown improved documentation and patient engagement, but combining PACT with navigation specifically in HCT survivors is a novel approach.

Eligibility criteria

Inclusion Criteria:

* Received a transplant for a hematologic malignancy
* Current age 18 years of age or older
* Currently 1 to 5 years after HCT
* Ability to speak and read English
* Has not completed all advance directives and/or had a goals of care (GOC) conversation with their physician
* Access to the internet (via computer, tablet, or mobile device)

Exclusion Criteria:

* Unable to provide informed consent
* Uncontrolled psychiatric conditions (confirmed through electronic health record \[EHR\] by study staff)
