What this trial studies
This study evaluates the effectiveness of a colorectal cancer (CRC) screening decision support tool integrated into primary care visits to increase screening completion rates among average risk patients aged 45-75. The tool is sent electronically to patients prior to their annual visit, guiding them through various screening options to help them make informed decisions. Patients can complete the tool via Patient Gateway or an iPad at the visit, and the results are discussed with their primary care clinician. Additionally, surveys will be conducted to assess patient and provider experiences with the decision-making process.
Who should consider this trial
Good fit: Ideal candidates are adults aged 45-75 who are overdue for colorectal cancer screening and are at average risk.
Not a fit: Patients who are at high risk for colorectal cancer will not benefit from this study.

Inclusion Criteria: Adults aged 45-75 overdue for CRC screening at average risk of CRC have an annual visit scheduled with participating practices Exclusion Criteria: * high risk of CRC
