What this trial studies
This observational study aims to implement a telemedicine-based cross-sector collaboration model to manage patients with frequent admissions due to decompensated heart failure. It will involve helping patients utilize telemedicine tools, facilitating teleconferences between general practitioners and cardiologists, and guiding community health workers in assisting patients. The study will compare readmission rates and quality of life for patients before and after the implementation of this model. Participants will include patients admitted with acute decompensated heart failure and atrial fibrillation, along with their healthcare providers.

Inclusion Criteria: Patients admitted with acute decompensated heart failure AND: * with previous admission with decompensated heart failure in the three-month period prior to the current admission or * who have two or more admissions in the 6 months prior to the current admission. Exclusion Criteria: * Patients who * have refused to share their data for health-quality projects * have already ongoing telemedicine management for heart failure
