What this trial studies
This project compares three delivery methods of team-delivered, exposure-based cognitive behavioral therapy (CBT) for youth with anxiety disorders and OCD across Rhode Island. Children and adolescents (ages 5–18) receive weekly exposure-focused sessions with an exposure coach plus monthly visits with a licensed provider, delivered either in-person, entirely via telehealth, or via a flexible combination. The Hybrid Type 1 effectiveness-implementation design measures clinical outcomes while also examining contextual factors that affect adoption, sustainment, and scalability in community behavioral health centers and a federally qualified health center. The study partners with Certified Community Behavioral Health Centers and a Federally Qualified Health Center to improve equitable access for historically marginalized families.
Who should consider this trial
Good fit: Ideal participants are outpatients aged 5–18 with a primary or co-primary DSM-5 diagnosis of an anxiety disorder or OCD, symptoms for at least three months, and a stable parent or guardian who can participate in treatment.
Not a fit: Patients with an active primary psychiatric condition requiring other immediate treatment, acute suicidality, concurrent psychotherapy, unstable psychotropic medication, or medical issues that prevent active participation are unlikely to benefit from this program.

Inclusion Criteria: * Age 5-18 inclusive * Primary or co-primary DSM-V diagnosis of anxiety or OCD * Symptom duration of at least 3 months * Outpatient care needed * Presence of a stable parent, or guardian, who can participate in treatment Exclusion Criteria: * Other primary or co-primary psychiatric disorder which requires initiation of other active current treatment * Acute suicidality * Concurrent psychotherapy * Chronic medical illness that would preclude their active participation in treatment * Treatment with psychotropic medication that is not stable
