What this trial studies

This prospective multicenter cohort enrolls adults (≥19 years) discharged from the ICU after more than 48 hours at two tertiary hospitals in Seoul, South Korea. Participants receive individualized daily rehabilitation sessions (up to 30 minutes) delivered through the Tuntun Rehabilitation Assistant smartphone platform, with a researcher assisting during the first five sessions. Physical and cognitive function, mobility, quality of life, platform usage, and safety events are measured at baseline, two weeks, hospital discharge, and during follow-up. Approximately 70 participants will be recruited to determine feasibility, usability, and preliminary clinical signals.

Who should consider this trial

Good fit: Adults aged 19 or older who were in the ICU for more than 48 hours, are medically stable at ICU discharge, own a smartphone, and can operate the digital platform are ideal candidates.

Not a fit: Patients with unstable medical conditions, severe cognitive impairment or communication barriers, formal contraindications to exercise, or inability to use a smartphone are unlikely to benefit from this intervention.

Why it matters

Potential benefit: If successful, the platform could make tailored rehabilitation easier to access and speed recovery of physical and cognitive function after ICU discharge.

How similar studies have performed: Early pilots and tele-rehabilitation programs for post-ICU patients have shown promising feasibility and functional gains, but robust randomized evidence is still limited.

Eligibility criteria

Inclusion Criteria:

* Age 19 years or older.
* Admission to the intensive care unit (ICU) for more than 48 hours.
* Hospitalization at Asan Medical Center or Seoul National University Bundang Hospital.
* Discharge from the ICU at the time of enrollment.
* Ownership of a smartphone and ability to operate a digital rehabilitation platform.
* Ability to understand the study procedures and provide voluntary written informed consent.

Exclusion Criteria:

* Medical conditions that contraindicate participation in rehabilitation exercise, as determined by the attending physician.
* Severe cognitive impairment or communication difficulties that interfere with understanding or following study instructions.
* Unstable medical conditions requiring ongoing intensive medical treatment.
* Inability to use a smartphone or digital platform independently.
* Refusal to participate or withdrawal of informed consent.
