We are testing a simple fasting blood screen to find familial high cholesterol in children ages 2 to 18.

What this trial studies

This interventional program uses a fasting blood sample to measure cholesterol and identify familial hypercholesterolemia in children aged 2–18. Parents and children provide informed consent before a minimum 8-hour fast and blood draw. Children with a known diagnosis of familial hypercholesterolemia or with recent-onset diabetes, profound hypothyroidism, acute nephrotic syndrome, or active cancer are excluded. The approach aims to improve diagnosis rates so affected children can start treatment early to reduce long-term cardiovascular risk.

Who should consider this trial

Good fit: Children aged 2 to 18 who can fast at least 8 hours and whose parents and child (as appropriate) can give informed consent, with no prior diagnosis of familial hypercholesterolemia.

Not a fit: Children already diagnosed with familial hypercholesterolemia or those with recent-onset diabetes (under one month), profound hypothyroidism, acute nephrotic syndrome, or active cancer are unlikely to benefit from this screening.

Why it matters

Potential benefit: If successful, earlier diagnosis could allow treatment to lower LDL levels and substantially reduce lifetime cardiovascular risk.

How similar studies have performed: Screening and cascade testing for familial hypercholesterolemia in children have been implemented elsewhere and have successfully identified many affected individuals for early treatment.

Inclusion Criteria:

* Age between 2 and 18 years
* Minimum 8-hour fasting
* Parents and children with signed informed consent

Exclusion Criteria:

* Known familial hypercholesterolemia
* Diabetes less than one month old
* Profound hypothyroidism
* Acute nephrotic syndrome
* cancer under treatment
