What this trial studies
The Russian Extreme Hypertriglyceridemia Registry is a prospective, multicenter observational effort enrolling patients with triglyceride levels ≥10 mmol/L confirmed on at least two occasions. It records demographic, clinical, laboratory, lipid profile, treatment, and available genetic testing data from routine medical records without changing patient care. Participants are followed annually or more often if clinically indicated, and data are pseudonymized for analysis. The registry seeks to describe real-world diagnostic approaches, treatment patterns, and molecular-genetic characteristics to inform future research and improve patient care.
Who should consider this trial
Good fit: Anyone in Russia of any age with confirmed extreme hypertriglyceridemia (triglycerides ≥10 mmol/L on at least two separate tests) who can give informed consent is eligible.
Not a fit: People whose very high triglycerides are clearly due to ongoing alcohol abuse or uncontrolled secondary conditions and who are not confirmed to have persistent extreme levels after treating the underlying cause may not benefit from the registry findings.
Why it matters
Potential benefit: If successful, the registry could help doctors better identify causes, personalize treatment, and reduce complications such as acute pancreatitis and cardiovascular events for people with extreme hypertriglyceridemia.
How similar studies have performed: Other national and international registries and genetic cohorts of severe hypertriglyceridemia have produced useful epidemiologic and genetic insights, although randomized interventional evidence remains limited.

Inclusion Criteria: * Patients with extreme hypertriglyceridemia (triglyceride levels ≥10 mmol/L), confirmed by at least two independent measurements. * Written informed consent from the patient or legal guardian. * No age restrictions. Exclusion Criteria: * Patients whose extreme hypertriglyceridemia is caused by alcohol abuse, uncontrolled diabetes, or other medical conditions, unless extreme hypertriglyceridemia is confirmed after controlling the underlying condition. * Lack of informed consent.
