What this trial studies
This observational cohort study aims to enroll patients experiencing persistent symptoms for more than 12 weeks following COVID-19. It utilizes a multidisciplinary care model to optimize medical diagnostics and therapeutic approaches for individuals suffering from post COVID-19 syndrome. The study seeks to understand the clinical trajectories and disease phenotypes associated with long COVID, which can manifest across various severity levels of the initial infection. Who should consider this trial
Good fit: Ideal candidates for this study are individuals who have experienced persistent symptoms for at least 12 weeks after a COVID-19 infection.
Not a fit: Patients who have not experienced symptoms for at least 12 weeks post-infection or have a low functional status may not benefit from this study.

Inclusion Criteria: * Individuals with persistent post COVID symptoms ≥ 12 weeks * post COVID functional status scale (PCFS) ≥ 2 Exclusion Criteria: * observation time after infection \< 12 weeks * PCFS \< 2
