What this trial studies

This observational cohort study aims to enroll patients experiencing persistent symptoms for more than 12 weeks following COVID-19. It utilizes a multidisciplinary care model to optimize medical diagnostics and therapeutic approaches for individuals suffering from post COVID-19 syndrome. Not a fit: Patients who have not experienced symptoms for at least 12 weeks post-infection or have a low functional status may not benefit from this study.

Inclusion Criteria:

* Individuals with persistent post COVID symptoms ≥ 12 weeks
* post COVID functional status scale (PCFS) ≥ 2

Exclusion Criteria:

* observation time after infection \< 12 weeks
* PCFS \< 2
