Regional anesthesia to reduce inflammation during vaso‑occlusive crises in sickle cell disease

Impact of Regional Anesthesia on Inflammatory Mechanisms During Vaso-occlusive Crisis in Sickle Cell Patients

Observational Centre Hospitalier Universitaire de la Guadeloupe · NCT07001189

This project will test whether giving regional (loco‑regional) anesthesia to adults hospitalized for sickle cell vaso‑occlusive crises reduces inflammatory markers and speeds recovery compared with usual care.

What this trial studies

This is an observational comparison of patients hospitalized for vaso‑occlusive crises who receive loco‑regional (regional) anesthesia versus those who do not. Blood samples will be analyzed for inflammatory markers such as microparticles, cytokines, and neutrophil extracellular traps at defined timepoints during crisis and resolution. The protocol aims to describe whether regional anesthesia is associated with a blunted inflammatory response and faster clinical improvement. The work is conducted at the Centre Hospitalier Universitaire de la Guadeloupe and uses routine clinical care decisions rather than randomized interventions.

Who should consider this trial

Good fit: Adults (over 18) with a vaso‑occlusive crisis who are hospitalized at the CHU de la Guadeloupe and meet local eligibility (not pregnant, no blood transfusion in past 3 months, and covered by French social security) are the intended participants.

Not a fit: Patients who are pregnant, recently transfused, lack French social security coverage, or who do not receive regional anesthesia during their hospitalization would not be included and therefore would not directly benefit from this study's findings.

Why it matters

Potential benefit: If successful, findings could support using regional anesthesia to reduce inflammation and pain duration during crises, possibly shortening hospital stays and lowering complications.

How similar studies have performed: Preclinical and in vitro studies show local anesthetics have anti‑inflammatory effects, but no clinical study has yet demonstrated cellular anti‑inflammatory effects of regional anesthesia specifically in sickle cell patients, so the approach is promising but clinically unproven in this population.
