Reducing unnecessary antibiotics for urinary tract infections by improving urine collection
A Randomized Controlled Trial of a Diagnostic Stewardship Intervention to Reduce Inappropriate Antibiotic Use for Urinary Tract Infections in Primary Care
This study will test whether a bilingual (English and Spanish) animated video and pictorial flyer can help adults in safety‑net primary care give cleaner urine samples to reduce contaminated cultures and unnecessary antibiotic prescriptions.

What this trial studies
The study implements a bilingual educational package—an animated video plus a pictorial flyer—focused on midstream clean‑catch urine collection for adults undergoing urine culture in safety‑net primary care. Participants at Baylor College of Medicine clinics will receive the materials and clinic staff will support the collection process. Researchers will track urine culture contamination rates and subsequent antibiotic prescriptions to see if contamination and inappropriate treatment decline. The approach emphasizes patient-facing, low‑literacy materials and stakeholder engagement to improve diagnostic accuracy.
Who should consider this trial
Good fit: Adults aged 18 or older receiving a urine culture in participating safety‑net primary care clinics who can consent and speak English or Spanish are eligible.
Not a fit: Patients with urinary catheters or those unable to read or sign consent or follow study procedures due to significant impairments are unlikely to benefit from this intervention.
Why it matters
Potential benefit: If successful, patients could have fewer contaminated urine tests and receive fewer unnecessary antibiotics, which may lower individual harms and community antimicrobial resistance.
How similar studies have performed: Prior interventions to reduce urine culture contamination have had mixed results, so this study uses bilingual animated and pictorial materials plus stakeholder engagement as a newer approach.
