This project will see how well first-line atezolizumab works and how safe it is for adults with stage IV non-small cell lung cancer whose tumors have PD-L1 ≥50% and no targetable mutations in routine care in Türkiye.

What this trial studies

This prospective, multicenter observational study will collect routine clinical data from about 150 patients treated with first-line atezolizumab across several Turkish hospitals. No protocol-mandated interventions are required; treatments, imaging, labs, and follow-up occur per each center's standard practice. Investigators will analyze survival, response, safety, and real-world treatment patterns using information abstracted from medical records. The goal is to describe how atezolizumab performs outside of controlled trials in a national cohort.

Who should consider this trial

Good fit: Adults (≥18) with stage IV non-small cell lung cancer, tumor PD-L1 expression ≥50%, no targetable driver mutations, who have been prescribed atezolizumab as first-line therapy and received up to three cycles by screening are eligible.

Not a fit: Patients with targetable mutations, those not receiving atezolizumab, pregnant or breastfeeding women, or those with known hypersensitivity to atezolizumab are unlikely to benefit from joining this project.

Why it matters

Potential benefit: If successful, this could show that first-line atezolizumab is effective and safe for adults with stage IV NSCLC and high PD-L1 expression in everyday clinical practice in Türkiye.

How similar studies have performed: Randomized trials have shown benefit for single-agent PD-(L)1 therapy in high PD-L1 advanced NSCLC, but prospective real-world evidence specifically describing first-line atezolizumab in this exact population is limited.

Eligibility criteria

Inclusion Criteria:

* Signed informed consent form.
* Diagnosis of stage IV non-small cell lung cancer.
* Male or female patients aged 18 years or older.
* Patients who have been prescribed atezolizumab in accordance with routine clinical practice and the locally approved indication in Türkiye.
* Patients who have received up to 3 cycles of atezolizumab at the screening visit.

Exclusion Criteria:

* Patients who are not receiving atezolizumab for the treatment of lung cancer according to standard of care and the approved indication.
* Known or suspected hypersensitivity to atezolizumab.
* Pregnant or breastfeeding women.
