This will see if a Quadro-Iliac Plane Block compared with wound infiltration reduces pain and opioid use in adults having elective single-level lumbar discectomy.

What this trial studies

This is a prospective observational comparison of two routine analgesic techniques used for elective single-level lumbar discectomy: the ultrasound-guided Quadro-Iliac Plane Block (QIPB) and local wound infiltration (WI). Patients receive standard care determined by their anesthesiologist without randomization, and investigators collect data during the first 24 hours after surgery. The primary outcome is cumulative opioid consumption in the first 24 hours, with secondary outcomes including pain scores, postoperative nausea and vomiting, rescue analgesic requirements, patient satisfaction, block-related complications, recovery parameters, and hospital length of stay. The study is conducted at Karabuk Training and Research Hospital in adult patients aged 18–80.

Who should consider this trial

Good fit: Adults aged 18–80 scheduled for elective single-level lumbar discectomy with ASA physical status I–III who can use patient-controlled analgesia and give informed consent.

Not a fit: Patients with chronic preoperative pain, prolonged opioid use, multilevel or revision spine surgery, contraindications to regional anesthesia, or severe organ dysfunction are unlikely to benefit from the findings.

Why it matters

Potential benefit: If successful, it could reduce opioid use and improve early pain control and recovery after lumbar discectomy.

How similar studies have performed: Related fascial plane blocks have shown benefit for postoperative analgesia in some spine and abdominal surgeries, but QIPB is a newer technique and direct comparative evidence versus wound infiltration is limited.

Eligibility criteria

Inclusion Criteria:

* Adults aged 18 to 80 years
* Scheduled for elective single-level lumbar discectomy
* ASA physical status I-III
* Able to use patient-controlled analgesia (PCA)
* Able and willing to provide written informed consent

Exclusion Criteria:

* History of opioid use for longer than 4 weeks
* Presence of chronic pain before surgery (e.g., migraine, fibromyalgia)
* Alcohol or substance dependence
* Known allergy or hypersensitivity to local anesthetics or opioids
* Significant organ dysfunction (e.g., severe hepatic or renal disease)
* Revision or multilevel spine surgery
* Contraindications to regional anesthesia
* Severe psychiatric disorders limiting cooperation (e.g., psychosis, dementia)
* Pregnancy or breastfeeding
* Hematologic disorders
