This will test whether extra breathing exercises and perioperative pulmonary physiotherapy can lower lung complications in adults having elective laparoscopic liver surgery.

What this trial studies

This is a prospective, multicenter, single-blinded randomized controlled trial enrolling about 326 adults scheduled for elective laparoscopic liver surgery. Participants will be randomized to receive enhanced perioperative pulmonary physiotherapy (preoperative counselling, breathing exercises, and peri/postoperative support) or usual care. The main outcome is the incidence of postoperative pulmonary complications using standard EPCO definitions, measured during the hospital stay and early follow-up. The trial is conducted at several university hospitals in Finland with standardized protocols and outcome assessment.

Who should consider this trial

Good fit: Adults scheduled for elective laparoscopic liver surgery who can give informed consent, complete questionnaires, and perform the prescribed breathing exercises are ideal candidates.

Not a fit: People having emergency or planned open liver surgery, those under 18, or those unwilling/unable to consent or perform the exercises are excluded and would not benefit from this protocol.

Why it matters

Potential benefit: If successful, the intervention could reduce postoperative lung complications and related ICU days, shorten hospital stays, and speed recovery after laparoscopic liver surgery.

How similar studies have performed: Previous trials have shown that preoperative respiratory physiotherapy reduces pulmonary complications after open major abdominal and laparoscopic colorectal surgery, but evidence specifically in laparoscopic liver surgery is limited.

Inclusion Criteria

* Patients who undergo elective, laparoscopic surgery of liver
* Patients who are able to provide informed written consent
* Patients capable of completing questionnaires at the time of consent
* Patients compliant in taking in preoperative pulmonary counseling and conducting the exercises

Exclusion Criteria:

* Age \<18 years
* Emergency surgery
* Planned open surgery
* Unwillingness to participate in the follow up assessment
* No informed consent
