This trial will try infusing material from your ostomy bag back into your unused intestine before reversal surgery to see if it helps the bowel recover faster for people with a diverting loop ileostomy.

What this trial studies

This Phase 1/2 pilot enrolls patients with a diverting loop ileostomy who are being evaluated for stoma takedown at UC San Diego. Participants receive antegrade infusion of ostomy effluent into the distal intestine in the weeks before their planned reversal operation to 're-train' the bowel. The study will track safety, feasibility, and early signals of faster return of normal bowel function and fewer post-operative complications. Findings will inform whether a larger trial is warranted.

Who should consider this trial

Good fit: Ideal candidates are people with a diverting loop ileostomy who are scheduled for takedown and who can give consent and perform or tolerate the stoma infusion procedure.

Not a fit: Patients who cannot consent, cannot perform or tolerate stoma infusion, or who have conditions that make reversal unlikely or unsafe are unlikely to benefit from this intervention.

Why it matters

Potential benefit: If successful, this approach could speed recovery of normal bowel function after ostomy reversal and reduce post-operative symptoms and complications.

How similar studies have performed: The idea of re-exposing unused bowel to enteric contents is relatively novel; small case series and physiologic studies suggest feasibility but randomized evidence is limited.

* Has a diverting loop ileostomy
* undergoing evaluation for stoma takedown

Exclusion Criteria:

* unable to perform consent
* unable to perform stoma infusion
