This project will see if combining standard brain imaging scores can predict 90-day recovery and vessel reopening in adults who had an acute ischemic stroke treated with clot-busting medicine and clot removal.

What this trial studies
This is an observational cohort of about 152 adults with acute ischemic stroke treated with both thrombolysis and mechanical thrombectomy, conducted at Centre Hospitalier de Gonesse over roughly nine months. Researchers will analyze isolated and combined aspects of routine brain imaging scores to see how well they predict functional outcome at 90 days (modified Rankin Scale) and angiographic recanalization. Patients with poor-quality imaging, primary hemorrhage, or lacking 90-day follow-up will be excluded. The aim is to identify simple, reliable imaging-based decision tools that can be used in routine clinical care.
Who should consider this trial
Good fit: Adults aged 18 or older with confirmed acute ischemic stroke who received both thrombolysis and thrombectomy and have clinical follow-up available at 90 days.
Not a fit: Patients with primary hemorrhagic stroke, very poor-quality imaging, or who die or are lost to follow-up before 90 days would not be included and are unlikely to benefit from the findings.
Why it matters
Potential benefit: If successful, this could give doctors simple imaging tools to better predict recovery and tailor post-stroke care.
How similar studies have performed: Prior studies show individual imaging scores such as ASPECTS can predict outcomes and combining score metrics is promising, but combined approaches are not yet fully validated.

Inclusion Criteria : * Patients aged 18 years or older * Confirmed acute ischemic stroke on imaging * Treated with both thrombolysis and thrombectomy * Clinical follow-up data available at 90 days (mRS score) Exclusion Criteria : * Primary hemorrhagic stroke * Early death or loss to follow-up without evaluation * Poor-quality imaging that cannot be analyzed
