What this trial studies

INSPIRE-PH is a multicenter, observational real-world registry that systematically collects longitudinal clinical and outcome data on children and adolescents diagnosed with pulmonary hypertension. Enrolled patients receive standard care from their treating physicians and no study-mandated interventions are imposed. The registry captures diagnostic, hemodynamic, treatment, and follow-up information to characterize disease course, treatment patterns, and prognosis over time. The cohort began prospectively in 2006 and cases have been reclassified according to contemporary World Symposium on Pulmonary Hypertension criteria to enable longitudinal comparisons.

Who should consider this trial

Good fit: Children and adolescents diagnosed with pulmonary hypertension before age 18 who can provide informed consent/assent and are able to attend longitudinal follow-up at participating centers.

Not a fit: Patients without a confirmed PH diagnosis, adults, or those unable to provide consent or commit to follow-up are unlikely to benefit from participation.

Why it matters

Potential benefit: If successful, the registry could improve understanding of pediatric PH progression and help refine risk scores and treatment approaches tailored to children.

How similar studies have performed: Other pediatric PH registries (for example TOPP and elements of REVEAL) have produced valuable natural-history and risk-stratification data, but pediatric therapeutic evidence remains limited.

Inclusion Criteria:

* Children and adolescents aged \<18 years at diagnosis
* Diagnosed pulmonary hypertension (PH) according to the prevailing diagnostic criteria at the time of enrollment
* Enrollment in the registry at participating centers
* Provision of informed consent by patients and/or legal guardians
* Availability for longitudinal follow-up

Exclusion Criteria:

* Inability to confirm PH diagnosis due to insufficient clinical or hemodynamic data
* Refusal or inability to provide informed consent
