This study tests whether using the oXiris hemofilter during continuous dialysis helps adults with septic shock and acute kidney injury survive and stabilize faster.

What this trial studies
This is a multicenter retrospective cohort study that uses a Target Trial Emulation framework to mimic a randomized comparison of CRRT started with the oXiris filter versus standard CRRT filters. Time zero is defined as CRRT initiation in ICU adults with septic shock and AKI, and outcomes include in-hospital mortality, fluid balance, and vasopressor requirements. Data come from five major South Korean medical centers and the Korea Sepsis Alliance registry, with IPTW used to adjust for baseline confounding and confounding by indication. The analysis covers patients treated between March 2021 and August 2025 and excludes early deaths, ESRD, and records with critical missing covariates.
Who should consider this trial
Good fit: Adults aged 19 or older admitted to the ICU with septic shock who develop AKI requiring continuous renal replacement therapy during the study period are the intended candidates.
Not a fit: Patients with end-stage renal disease, those who die or have life support withdrawn within 48 hours of ICU admission, or patients who do not require CRRT are unlikely to benefit from the findings.
Why it matters
Potential benefit: If effective, oXiris use could lead to faster hemodynamic stabilization and lower in-hospital mortality for patients with septic shock on CRRT.

Inclusion Criteria: * Adults aged 19 years or older. * Admitted to the ICU with septic shock (Sepsis-3 criteria) between March 2021 and August 2025. * Required Continuous Renal Replacement Therapy (CRRT) due to acute kidney injury (AKI). Exclusion Criteria: * Death or withdrawal of life-sustaining therapy within 48 hours of ICU admission. * Patients with End-Stage Renal Disease (ESRD). * Patients with missing critical covariates.
