This 12-month pilot will test whether a nurse-led program using a smartphone app, MyChart messages, and phone coaching helps adults on Medicaid who have type 2 diabetes and use insulin better manage their blood sugar.

What this trial studies
Guided by the Self- and Family Management Framework, this 12-month pilot delivers a nurse-led, app-based behavioral program to Medicaid-enrolled adults with type 2 diabetes on insulin. The intervention combines MyChart education and goal-setting, a clinician–patient collaborative problem-solving action plan, and remote glucose monitoring via the mySugr app. Nurses provide messaging and phone-based support to reinforce self-management behaviors and identify when treatment adjustments may be needed. The study measures changes in self-management behaviors and glycemic control compared with usual care.
Who should consider this trial
Good fit: Ideal candidates are adults (18+) with type 2 diabetes who are on insulin, have an elevated A1C, are Medicaid or dual eligible, and can read English and use an Android or iOS smartphone.
Not a fit: Patients without a smartphone or smartphone skills, who are pregnant, have significant cognitive impairment, or do not read English are unlikely to benefit from this intervention.
Why it matters
Potential benefit: If successful, the program could improve daily diabetes self-management and lower A1C for Medicaid patients by combining digital tracking with nurse support.
How similar studies have performed: Prior nurse-led and app-based remote-monitoring programs have shown modest improvements in self-management and A1C in some populations, but results vary and Medicaid groups have been less frequently studied.
Eligibility criteria

Inclusion Criteria: * A1C \>7 for age 18-64, A1C\>8 for age 65 or older within 12 months prior baseline * A1C\>7 for age 18-64, A1c\>8 for age 65 or older at baseline * Medicaid recipient or dual Medicaid and Medicare recipient * Age 18 or older * Type 2 diabetes * On insulin therapy * Owning and using Android or iOS smartphone for at least 6 months * English proficiency * Adequate vision to read text messages on their current smartphone * Smartphone use proficiency. Smartphone proficiency will be self-reported and determined by telephone as the ability to use a smartphone in ways other than emailing, texting, and making phone calls. Exclusion Criteria: * Pregnancy * Patients with cognitive impairment who will be screened with the Six-Item Screener to Identify Cognitive Impairment Among Potential Research Subjects.
