This study tests whether a nurse-led transitional care program helps Tanzanian stroke survivors and their caregivers feel more prepared for discharge, manage the condition better, and have a higher quality of life.

What this trial studies
This is an observational follow-up of stroke survivors and their family caregivers who are already taking part in a nurse-led transitional care program at Muhimbili University of Health and Allied Sciences in Dar es Salaam. Participants (with mild-to-moderate stroke and the ability to communicate and use a mobile phone) will be followed for six months to track discharge preparedness, self-management behaviors, and quality-of-life outcomes. Clinical staff with at least six months' stroke experience are included as part of the program context, and outcomes are collected as part of routine care without randomization. The study uses standardized measures such as NIHSS, Modified Barthel Index, Modified Rankin Scale, and MoCA to describe participant eligibility and baseline function.
Who should consider this trial
Good fit: Ideal participants are adults admitted to the stroke unit with a confirmed primary stroke, mild deficits (NIHSS <6), reasonable functional status (MBI >9, mRS <5), MoCA >14, who live with a family caregiver, can communicate, read and write, and have a mobile phone.
Not a fit: Patients with more severe strokes, significant cognitive impairment, no caregiver support, inability to communicate or use a phone, or those outside the Dar es Salaam treatment area are unlikely to benefit from this specific program.
Why it matters
Potential benefit: If successful, the program could improve discharge readiness, self-care after hospital discharge, and overall quality of life for stroke survivors in this setting.
How similar studies have performed: Nurse-led transitional care programs have shown benefits for discharge planning and self-management in other countries, but evidence from Tanzania and similar sub-Saharan African settings is limited.

Inclusion criteria * Clinical nurses and physicians with six months of working experience in stroke care * Clinical nurses and physicians with a minimum of diploma in their professions. * Stroke survivors with 18 years old and above * Stroke survivors admitted in the stroke units * Stroke survivors with primary diagnosis of stroke confirmed by brain CT/MRI * Stroke survivors who undergo usual discharge process * Stroke survivors who live with their family caregivers * Stroke survivors who have mobile phones * Stroke survivors who can read and write * Stroke survivors who are able to communicate * Stroke survivors with National Institutes of Health Stroke Scale (NIHSS) \< 6 * Stroke survivors with Modified Barthel Index (MBI) \> 9 * Stroke survivors with Modified Rankin Scale (mRS) \< 5 * Stroke survivors with Montreal Cognitive Assessment Test (MoCA) \> 14 * Stroke survivors who are expected to stay in the ward for 3-5 days, * Stroke survivors who are expected to survive for 3 months * Family caregivers with mobile phones * Family caregivers who can read and write * Family caregivers who are able to communicate * Family caregivers who live with the patient after stroke Exclusion criteria * Healthcare providers who will be on leave during the study period. * Stroke survivors with previous stroke who are not admitted in stroke units * Stroke survivors who are discharged against medical advice * Stroke survivors who have end-stage organ failure * Stroke survivors who have family caregivers * Stroke survivors who can't read/write * Stroke survivors who have no mobile phones. * Family caregivers without mobile phone that is accessible
