What this trial studies
This study evaluates a novel surgical protocol for treating periodontal disease by making a single incision in the midline of the interproximal area. The approach aims to preserve granulation tissue, which has regenerative potential, while comparing outcomes to a control group receiving a modified Kirkland flap. Clinical parameters such as bleeding on probing and clinical attachment levels will be assessed at surgery and after 12 months. The trial includes 50 participants, divided into two groups, to determine the effectiveness of the new technique.
Who should consider this trial
Good fit: Ideal candidates are individuals with Stage III or IV periodontitis who have unresolved deep pockets after non-surgical treatment.
Not a fit: Patients with systemic diseases contraindicating periodontal surgery or those with Stage I or II periodontitis may not benefit from this study.

Eligibility criteria

Inclusion Criteria: * Stage III or IV periodontitis, including all grades. * Unresolved deep pockets (probing pocket depth \[PPD\] \>5 mm + BoP) 4 to 6 weeks after non-surgical treatment. * Interproximal plaque index \<35% maintained during periodontal treatment and maintenance. * Adherence to periodontal maintenance appointments. Exclusion Criteria: * Systemic disease contraindicating periodontal surgery. * Teeth with incorrect endodontic treatment or restoration. * Stage I or II periodontitis.
