What this trial studies

More Health, Less Diabetes is a three-arm randomized controlled trial comparing supervised online exercise, supervised in-person exercise, and a control group in adults at high risk for type 2 diabetes. The intervention lasts 12 months with six months of supervised exercise (three times per week) and concurrent educational sessions, followed by a six-month follow-up. The primary outcome is 2-hour glucose from an oral glucose tolerance test, with secondary outcomes including HbA1c, fasting glucose, insulin resistance indices, and cost-effectiveness analyses. The trial is delivered from the Faculty of Human Kinetics in Lisbon and includes both clinical and economic endpoints to inform scalable prevention strategies.

Who should consider this trial

Good fit: Adults at high or very high risk on the FINDRISC questionnaire, without diagnostic criteria for diabetes on lab testing, who have internet access, can attend scheduled sessions, and are willing to make lifestyle changes.

Not a fit: People who already meet diagnostic criteria for diabetes, are pregnant, cannot use digital devices, or have exercise stress test abnormalities that prevent safe participation may not benefit or be eligible.

Inclusion Criteria:

1. High or Very High Risk according to the FINDRISC questionnaire;
2. Analytical confirmation of no diagnostic criteria for Diabetes, following a type 2 Diabetes risk assessment;
3. Committed to making lifestyle changes to manage diabetes;
4. Internet access;
5. Availability to participate in the program as required (schedule, location);

Exclusion Criteria:

1. pregnancy (self-report/medical record);
2. Inability to use digital devices
3. Presence of abnormalities on exercise stress test.
