What this trial studies
This pilot clinical trial aims to refine and test a mobile health intervention designed to improve medication adherence among adolescents and young adults with chronic health conditions. The study will first gather feedback from an initial cohort to tailor the intervention to better meet patient needs. Following this, a larger group will be randomized to receive either automated text reminders or an adaptive cell phone support system, which includes personalized coaching based on individual responses. The goal is to determine which approach is more effective in promoting adherence to medication regimens.

Inclusion Criteria: * Children's Hospital Los Angeles patient * between 15-20 years old * taking at least one oral medication per day for a chronic health condition * English speaking * demonstrating sufficient cognitive capacity to engage in the assent/consent process and study procedures * has a cell phone There is no exclusion criteria.
