What this trial studies
This study aims to develop an automated mobile education system for patients with heart failure, providing continuous education from hospitalization through the post-discharge period. The intervention will utilize the 'Line' platform to deliver personalized disease care education over three months, focusing on improving patients' knowledge, self-efficacy, and quality of life. By integrating this technology, the study seeks to reduce the burden on nursing staff while enhancing patient self-management and reducing hospital readmissions.

Inclusion Criteria: * Patients with a diagnosis of heart failure * Patients with age \>= 20 years old * patients with clear consciousness * Patients who can communicate with by Chinese or Taiwanese, and are willing to participate this study Exclusion Criteria: * patients who are long-term bedridden, or living dependent of others * patients who have severe psychological disease, or severe cognitive dysfunction * patients without utilizing smartphone or internet
