What this trial studies
The Smart About Meds (SAM) project aims to address medication nonadherence and the use of potentially inappropriate medications among patients discharged from hospitals. This randomized controlled trial will involve 3,250 patients from various medical units at the Royal Victoria Hospital and the Montreal General Hospital. Participants will be randomly assigned to receive either usual care or the SAM mobile application, which features a continuously updated medication list and pill images to enhance patient understanding and adherence. The effectiveness of the SAM app will be evaluated in improving medication management post-discharge.
Who should consider this trial
Good fit: Ideal candidates include adults aged 18 and older who are discharged from specific hospital units and have at least one medication prescribed at discharge.
Not a fit: Patients discharged to rehabilitation centers or long-term care facilities may not benefit from this study.
Why it matters
Potential benefit: If successful, this intervention could significantly reduce hospital readmissions and improve patient outcomes through better medication adherence.
How similar studies have performed: Other studies have shown promise in using mobile applications for medication management, indicating potential success for this approach.
