Low blood pressure in the recovery room after general anesthesia

Incidence of Hypotension in the Post-anesthesia Care Unit (PACU).

Observational Tomas Bata Hospital, Czech Republic · NCT07022210

This project will test how often adults who have general anesthesia develop low blood pressure in the post-anesthesia recovery room and what factors are linked to it.

What this trial studies

This is an observational audit of adults undergoing procedures with general anesthesia who spend time in the post-anesthesia care unit (PACU). Hypotension in the PACU is defined as systolic blood pressure below 90 mmHg or a drop greater than 20% from baseline, and blood pressure readings will be extracted from routine clinical records. The study will measure the frequency of PACU hypotension and use patient, anesthesia, and surgical variables to identify factors associated with its occurrence. Patients with incomplete records, direct ICU transfers, or preexisting unstable hemodynamics are excluded to preserve data quality.

Who should consider this trial

Good fit: Adults (age 18+) who undergo procedures under general anesthesia and remain in the hospital's PACU with consent for data use are eligible to participate.

Not a fit: Patients transferred directly to the ICU after surgery, children, those with preexisting resistant hypotension, or cases with missing PACU blood pressure records are unlikely to benefit from this study's findings.

Why it matters

Potential benefit: If successful, the findings could help clinical teams spot and treat low blood pressure earlier in the recovery room to reduce the risk of organ hypoperfusion after surgery.

How similar studies have performed: While intraoperative hypotension has been well described, PACU hypotension has been less studied and similar observational audits have reported variable incidence and identified several contributory factors.

Eligibility criteria

Show full inclusion / exclusion criteria
Inclusion Criteria:

* patients over 18 years old
* patients undergoing a procedure under general anesthesia
* patients who have consented to data processing
* postoperative stay in PACU

Exclusion Criteria:

* patients younger than 18 years old.
* patients transferred directly to the ICU/Anesthesiology and Resuscitation Department after surgery without staying in the recovery room.
* patients with incomplete medical documentation or missing blood pressure values in the recovery room.
* patients with a pre-existing diagnosis of resistant hypotension or unstable hemodynamics before surgery
* patients with a history of long-term use of anxiolytics, antipsychotics, or other psychotropic medications that may affect cognitive function, emotional state, or predictive outcomes
* patient refusal to participate in the study (including cases where they initially consented), with an emphasis on respecting patient autonomy
