What this trial studies

This randomized clinical trial at King Edward Medical University (Mayo Hospital), Lahore, will assign 64 adults undergoing Milligan-Morgan hemorrhoidectomy to receive either topical lignocaine or 0.2% GTN ointment applied postoperatively. Pain intensity will be measured at regular intervals using a visual analog scale (VAS) and wound healing will be monitored during recovery. The study compares the two topical agents for immediate postoperative analgesia and effects on wound healing and patient comfort. The overall study period is planned for about 3–6 months after protocol approval, with data collected during the postoperative follow-up.

Who should consider this trial

Good fit: Adults aged 18–60 with Grade III or IV hemorrhoids (ASA I–II) scheduled for hemorrhoidectomy who are not pregnant and have no allergy to lignocaine or GTN are ideal candidates.

Not a fit: Patients who are pregnant or lactating, have cardiovascular disease, take nitrates or calcium-channel blockers, have other perianal pathology requiring different surgery, or are allergic to either medication are unlikely to qualify or receive benefit.

Why it matters

Potential benefit: If successful, the preferred ointment could provide faster and better pain relief, improving comfort and recovery after hemorrhoid surgery.

How similar studies have performed: Previous smaller studies have shown that both topical lignocaine and GTN can reduce post-hemorrhoidectomy pain, but direct head-to-head comparisons are limited.

Inclusion Criteria:

* Age between 18 - 60 years
* Both genders
* Grade III and IV hemorrhoids diagnosed on the basis of history and clinical examination by consultant Surgeon.(Grade III Hemorrhoid manually reduced by patient with finger and Grade IV is prolapsed)
* ASA (American society of Anesthesia) grades I, and II

Exclusion Criteria:

* Previously reported allergy or reaction to lignocaine and GTN
* Pregnancy or lactation
* Concomitant perianal pathology warranting surgery
* Cardiovascular disease
* Patient taking Nitrates or Calcium channel blocker
