What this trial studies

This clinical trial aims to evaluate the effectiveness of a 6-month lifestyle intervention for patients with chronic kidney disease (CKD) stages 3-5. A total of 120 participants will undergo a self-controlled trial, which includes a 3-month control phase, a 6-month intervention phase, and a 3-month maintenance phase, with no interventions during the control and maintenance periods. The intervention will involve a multidisciplinary team providing exercise training and dietary guidance, focusing on improving physical fitness and immune function. Additionally, participants in earlier stages of CKD and healthy subjects will be assessed for baseline fitness and immune function without receiving any intervention.

Who should consider this trial

Good fit: Ideal candidates for this study are patients with chronic kidney disease in stages 3-5 who have been on hemodialysis for more than 6 months and are stable during dialysis.

Not a fit: Patients with unstable cardiac conditions or those with comorbidities that contraindicate exercise may not benefit from this study.

Eligibility criteria

Inclusion Criteria:

* patients with CKD in stages 3-5
* patients who are on HD for longer than 6 months
* under the permission of their nephrologist
* adequately dialyzed (most recent Kt/V \> 1.2) and stable during dialysis in the past 3 months

Exclusion Criteria:

* occurrence of hyperkalemia in the past 3 months
* comorbid medical, physical, and mental conditions that contraindicate exercise
* unstable cardiac conditions (eg, unstable angina, heart failure or symptomatic severe aortic stenosis, etc.)
* disabling orthopedic and neuromuscular diseases
