This trial will try adding a free gingival graft to non-surgical peri-implantitis care to see if increasing keratinized mucosa helps patients with less than 2 mm of mid‑buccal keratinized tissue heal better.

What this trial studies
Patients with peri-implantitis and inadequate keratinized mucosa receive standard non-surgical therapy, then one month later are assigned either to receive a free gingival graft (FGG) to increase keratinized mucosa or to continue supportive periodontal care and regular follow-up. The FGG is harvested from the palate and placed to augment width and thickness of peri-implant keratinized mucosa. Clinical and radiographic outcomes including probing depth and signs of inflammation are measured, with the primary endpoint being change in probing depth at 12 months. Eligible participants are systemically healthy non-smokers with mid‑buccal keratinized mucosa under 2 mm, and those taking medications that impair healing or who are pregnant or lactating are excluded.
Who should consider this trial
Good fit: Adults with peri-implantitis who are non-smokers, otherwise healthy, and have less than 2 mm of mid‑buccal keratinized mucosa are the intended candidates.
Not a fit: Patients who smoke, have systemic conditions or take medications that impair periodontal healing, are pregnant or lactating, or already have adequate keratinized mucosa are unlikely to benefit from this adjunctive procedure.
Why it matters
Potential benefit: If successful, adding a free gingival graft could reduce probing depths and improve soft tissue stability around implants, lowering the risk of further bone loss and implant failure.
How similar studies have performed: Previous studies offer mixed results on the benefit of increasing keratinized mucosa for peri-implant health, so adjunctive free gingival grafting remains promising but not conclusively proven.
